New York: Update 2/23/21

Healthcare/Homecare workers, appointments now available.
Some hospitals have extra vaccines for home health care workers - most just require a phone call to schedule appointments and some are offering walk-in appointments. Due to the high positivity rate, effective 2.5.21, Yankee Stadium will be offering the vaccine to all residents who fall within the 1a and 1b eligibility categories:
Connecticut: Update 2/23/21
*currently 65+ can be vaccinated. Those 55+ will be eligible effective March 1 and the last group of 16-34 will begin May 3*
For healthcare/homecare workers, we have registered your employment with the state and the local CDC will contact you directly via email with a link to register.
For clients and their families 65+ visit ct.gov/covidvaccine to schedule an appointment through the web-based Vaccine Administration Management System (VAMS).
Be sure to complete any pre-vaccine forms online and bring your company issued ID with you to your appointment.

New Jersey:
For healthcare/homecare workers - you are eligible now!

Be sure to complete any pre-vaccine forms online and though not required at this time, please consider bringing your company issued ID with you to your appointment.
North Carolina:
All healthcare/homecare workers and clients/families 65+, as well as adults (16-64) that have high risk medical conditions - vaccines are now available! To begin the eligibility and appointment process, please visit the link below or call the hotline.

All healthcare/homecare workers residing in Cook County are eligible now!
Please be on the lookout for a text message from Premier with a link to register for an appointment to receive the vaccine. Please be aware that there is no charge for the vaccination.
The vaccine is being offered on a priority basis. When you receive the link, do not share it with anyone that is not a Premier employee residing in Cook County.
For those choosing the Chicago department of health locations from the link, an account will need to be created. Choose the location, pick a date and time from what’s available in the drop down for that location, click register and complete the consent form. Those appointments are immediately booked.
For the Oak Street Health locations from the link provided, an informational form will need to be submitted to Oak Street Health and their medical staff will contact you to schedule an appointment. Please be patient, there is high demand for all these locations.

Florida
For healthcare/homecare workers, the Florida Department of Health (DOH) in Collier County, in partnership with the Healthcare Network, will provide COVID19 vaccine administration beginning Monday, January 11, 2021, at North Collier Regional Park - Athletics Field.
Appointments for the COVID19 vaccine will be made via Eventbrite. Please visit CollierCountyCOVIDVaccines.Eventbrite.com.
Publix has also opened vaccination opportunities for eligible residents.
The state has added a requirement to document residency or proof of employment status as a home care worker. Please be sure to bring your Premier issued ID with you to your appointment.
Not eligible yet? Pre-register at the link below to “get in line” for a vaccine in your county so you can be contacted via phone, text or email (your preference) when appointments become available.
Massachusetts: Udpated 2/23/21
*Officially opened up to 65+ and high risk conditions while also allowing the companion to get vaccinated w/appointment if accompanying someone 75*
Effective 1/21/21, all healthcare/homecare workers are eligible for the vaccine. A comprehensive checklist for the process can be found at this here and a location map can be found here.
You must complete an attestation form before arriving to your appointment.
Home care workers are urged to check mass vaccination sites, such as Gillette Stadium, Fenway or local pharmacies for appointments. There are several available!
